Jeremiah Bradford PT, DPT, is a Doctor of Physical Therapy with over 8 years of educational and clinical experience in the field of physical rehabilitation.  Born and raised in Lum, Michigan, he received his Bachelor of Science degree with a major in Exercise Science from Western Michigan University in Kalamazoo, Michigan.  He then went on to obtain his Clinical Doctorate of Physical Therapy from the University of Mary in Bismarck, North Dakota. After graduation he moved to Florida to be closer to his family and get out of the cold weather.

He has always had a passion for human movement, functional capabilities and the body’s ability to heal itself after sustaining an injury. He devotes himself to his passion everyday to help improve the lives of others through improved functional movement. He has a particular interest in working with the senior population.  Jeremiah’s philosophy is that age is truly just a number, you can accomplish anything you set your mind to at any stage of life.

With so many of the senior population choosing to age in place, he saw a need in the local community for home based/mobile therapy and knew he had the knowledge, experience and capability to fill this need.  Whether you live in an assisted living community, or in your home, you should have access to the highest quality and evidence based outpatient therapy.  He believes that ageism has no place in Florida and as a therapist he treats the whole person, not a condition or diagnosis.

He specializes in working with people that have Parkinson’s disease. He obtained his certification with LSVT BIG® in 2019.  Through this program, he has helped many people regain their function and independence. He continues to support the Parkinson’s community through health, wellness and support programs.

Jeremiah received his Lymphedema certification including Advanced Lymphedema Management in 2021 which covers the entire spectrum of Lymphedema management. This certification gives a therapist a thorough knowledge and understanding of all aspects of Complete Decongestive Therapy, which is considered the "gold standard" treatment for Lymphedema.

Jeremiah received his Dry Needling certification in 2023.  Dry Needling is a form of physical therapy which is used to relieve pain, improve movement and flexibility, and promote healing.

He founded Unified Rehabilitation, PLLC which is uniquely qualified to provide the highest standard of care possible offered in central Florida.  He is proud to lead a team of therapists that share the same values and mission in patient care.  Jeremiah and his team provide everyone with an individualized plan of care tailored to their physical needs and equally as important to address their social and mental needs as well.

Jeremiah believes in strong community involvement and building long lasting relationships.  He is a member of the American Physical Therapy Association and Florida Physical Therapy Association.  In addition, he is an active member of the Ocoee Lions Club, the West Orange Chamber of Commerce and the Horizon West Rotary Club.  He has done volunteer work with the Breast Cancer Walkathon, Strong kids campaign and Home Run Heroes.

In his free time, Jeremiah enjoys camping, biking, traveling and spending time with family, including his beloved dog Thor.

Jeremiah and his team look forward to many years of providing service to the community.

Although she enjoys working with all patients, she has a particular interest in working with older adults in order to help them maintain or regain their independence as they age.

Anh currently leads exercise classes at an Assisted Living Facility in order to help older adults get their daily dose of physical activity to keep them strong, healthy, and active. She also participates in balance and falls screenings to help individuals recognize their risk and provide information to prevent falls from occurring. She believes in developing a strong patient-therapist relationship in order to develop a plan of care that will help them achieve their individualized goals. She excels at helping patients realize what they’re capable of by consistently challenging them with fun and creative exercises involving her patient’s interests to make it more enjoyable and meaningful.

Anh knew she wanted a career in the healthcare field and was drawn to physical therapy because she wanted to make a positive change in people’s lives and improve their quality of life. It’s extremely rewarding to Anh to be able to help individuals get back to whatever it is they wanted to do, or help them maintain as much independence as possible. She enjoys having the ability to actively be a part of a person’s rehabilitation process and sees them through it every step of the way. She also likes meeting new people and having the opportunity to educate them about their health and how to stay active in order to live life to the fullest!

During her free time, she likes to travel and explore new places, play tennis and soccer, relax at the beach, and spend time with her friends and family. Lastly, she is an avid dog lover and always enjoys seeing pictures or talking about dogs!

Imani Aird, COTA/L

Occupational Therapist Assistant

EDUCATION & AFFILIATIONS:

  • Associate of science degree - Polk State College
  • Bachelor of arts degree in Behavioral Psychology - Florida International University
Imani graduated from Polk State College where she earned her associate of science degree as an occupational therapist assistant. She also earned her bachelor of arts degree in behavioral psychology at Florida International University. She is currently pursuing her master of science degree in occupational therapy at Barry University. She has a special interest in stroke rehabilitation and working with patients who have Alzheimer's and dementia.

Imani became interested in therapy when her aunt suffered from a stroke when she was 38 years old. Imani was 15 at the time and saw the involvement of physical therapists, occupational therapists and speech therapists in her aunt's recovery.  She decided to research how therapy can help a patient recover from a stroke and other illnesses. Around this time, the high school she attended had every student take a career personality test. The top two careers chosen for her were dance teacher and occupational therapist!

Imani wanted to make a bigger impact in the lives of adults and the older adult population so she chose the healthcare route and began her journey into therapy. She used to be a dancer and has played the piano for most of her life. She gave dance lessons to younger girls at the studio that she attended in south Florida. She has been able to use her passion for music and dance after seeing how much it has a positive impact when incorporated into therapy for those with Alzheimer's and dementia.
